Health Check
Health Check is a federally mandated program that is intended to prevent chronic or debilitating conditions in low income families. At our clinic, our nurses identify "at risk" recipients who are not currently receiving preventative care and provide a health check screening and assessment, educate these individulas on matters of health and help them to establish a relationship with a local health care provider.
