Health Check Health Check is a federally mandated program that is intended to prevent chronic or debilitating conditions in low income families, We identify recipients who are not receiving preventative care; seek recipients who are "at risk", educating them on matters of health and helping them establish a relationship with a health care provider, and provide health check screenings, assessments and referrals.
Prenatal Care Coordination The Prenatal Care Program is designed to support women throughout their pregnancy and into the baby's first two months of life. Services include ongoing risk assessment (including alcohol, drug and smoking), health and nutrition education, and case management. This is an unduplicated, wrap around service that augments the care provided by a physician. The program targets teens and adult women eligible for medical assistance who are found to have a high risk for an unfavorable pregnancy outcome.
Breastfeeding Support Services Our breastfeeding support services inform, educate and support women who have made the decision to breastfeed their babies. We believe that breastfeeding is the healthiest choice for mothers and infants.
Contracted Services by Registered Dietitians Our agency provides qualified dietitians to outside agencies needing services, Contracts include Green County Council on Aging meal sites, nursing homes, Rock, Walworth and Dane county Head Start and Early Headstart programs, assisted living centers, staff relief to local hospitals and special needs populations.
